Academic Forgiveness Form – when you have been out of Haskell for 2 years or more, you can get your D or F grades to not count towards your cumulative GPA.
Change of Major or Advisor – you can use this form to change your major or your advisor
Petition to Graduate – you can use this form to let the Office of the Registrar know you want to graduate. Usually completed the semester before you plan on graduating.
KU/Haskell Exchange Form  – This form is to apply for the KU Exchange Program.
Release of Information (FERPA) Form – This form allows you to change or remove who can request information about you.

Instructor Forms:
Change of Grade Form – Instructors can use this form to correct a grade that was submitted incorrectly or if a student has completed their incomplete contract.
Incomplete Contract – Instructors will use this form to have a contract with a student on coursework needing to complete due to unforeseen circumstances out of the student’s control.
